How to set up an appointment

To set up an appointment, call 888-339-0588. You can also start the pre-appointment process online by CLICKING HERE. Please be ready to provide the name of your primary or referring physician, as well as the doctor with whom you would like to schedule your appointment. Also, please have your insurance information and diagnosis available, so that we can set up a consult with the physician.

Insurance

We accept all major insurance plans and will submit bills to your primary and secondary insurance carriers. Financial worries should not disrupt your treatment and our staff will work with you to explain coverage, any co-payments and help you plan financial assistance if needed. All major insurances accepted. Once we receive your insurance information, we will verify your coverage. Depending on your coverage, we will provide the insurance company with the necessary information for you to receive a referral or full authorization for your office visits, simulation, and/or treatment appointments.
